
WISEWOMAN
WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation) is a program created to help women understand and reduce their risk for heart disease and stroke by providing services to promote lasting heart-healthy lifestyles. WISEWOMAN helps integrate innovative and evidence-based approaches to heart disease and stroke prevention within health care systems and throughout communities.
Quality Insights has partnered with the Delaware Division of Public Health to encourage and assist providers in becoming a WISEWOMAN site. The WISEWOMAN program enables qualifying women to receive free screenings and counseling about their risk for heart disease and stroke. Women are then supported as they participate in evidence-based lifestyle programs, individual health coaching, or referred to other community resources. The services are all designed to promote lifelong heart-healthy lifestyle changes. To learn more, please download the

Patient Eligibility Requirements for WISEWOMAN Services
* Low-income women
* Uninsured women
* Underinsured women
* Women ages 40 to 64 years old
* Women who qualify for or are currently enrolled in
the Screening for Life program
Guidelines for Participation as a WISEWOMAN Site
* Be a Screening for Life provider site
* Provide care for low-income, uninsured, and underinsured
women ages 40 to 64
* Conduct risk reduction counseling
* Complete Integrated Office Visit with patients
* Work with Quality Insights Health Coach to refer patients to
Healthy Behavior Support Services including National
Diabetes Prevention Program, Self-Management Programs,
and the DE Quitline, among others.

Provides invaluable support for women who are at high
risk for cardiovascular disease

Supplies reimbursement for office visits and lab tests for
WISEWOMAN patients
*
Grants access to Quality Insights Health Coach who
reminds patients of appointments, refers patients to
evidence-based healthy behavior support services, and
follow up with patients regarding their health
*
Can help to increase multiple UDS and Medicaid Adult
Core eCQMs including:
a. Preventive Care and Screening: Body Mass Index (BMI)
Screening and Follow-Up Plan CMS69v9.
b. Preventive Care and Screening: Tobacco Use: Screening
and Cessation Intervention CMS138v9.
