Join co-hosts Joe Pinto and Lamartz Brown, both Practice Transformation Specialists at Quality Insights, as they sit down with the folks at Wayne Memorial Community Health Centers (WMCHC) to discuss it’s Telehealth at Home Program. This program is designed to address the clinical and social needs of patients who are experiencing socio-economic disparities that effect clinical outcomes.  The program focuses on managing patients with severe chronic conditions that require frequent assessment and management and is an extension of the outpatient office visit.

Our guest speakers from WMCHC include Norma Nocilla, BSHA, LPN, CPHQ – Director of Clinical Operations and Quality, James G. Cruse, MD – Chief Medical Officer of WMCHC and Medical Director of Wayne Memorial Hospice, and also Michelle Cook, RN – Telehealth Home Nurse.

Quality Insights Practice Transformation Specialists Joe Pinto and Lamartz Brown are joined by Amanda Vommaro and Walter Wanas from The Wright Center, located in Scranton, PA, to discuss how their practices use the PRAPARE screening tool to address social determinants of health with their patients.  They explain how the clinicians use this tool to assess each patient’s risks and experiences, and then how they use the responses to direct care.

Amanda Vommaro is the Director of Patient Centered Services and Outreach/Co-Director of Patient and Community Engagement, and Walter Wanas is the Director of Lifestyle Modification and Preventative Medicine.

Join host Joe Pinto, Quality Insights Practice Transformation Specialist, as he interviews Sue Chute, CEO of Multi-Cultural Health Evaluation Systems, Inc. (MHEDS) of Erie, PA. MHEDS is a Practice Champion in Quality Insights' hypertension and diabetes prevention and control program.

Joe and Sue discuss the Target:BP Recognition Program, which aims to improve blood pressure control rates across the country, and how practices are recognized for their efforts in effectively addressing hypertension within their patient populations. Sue also shares some of the strategies MHEDS employs to help patients better control hypertension, some of the barriers their team encountered, and lessons learned.
